Phyxit Physio and Injury Clinic (Phyxit, we, us) is committed to protecting your privacy. We handle personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records and Information Privacy Act 2002 (NSW).
What we collect
To provide physiotherapy services we collect your name, date of birth, contact details, emergency contact, Medicare, health fund, workers compensation or CTP claim details where relevant, and health information: your history, the reason for your visit, assessment findings, treatment notes, imaging and reports, and correspondence with your doctor or other health professionals. We collect this from you directly, and, with your consent or where the law allows, from referrers, insurers and other treating practitioners.
How we use it
We use your information to assess and treat you, to communicate with you about your care and appointments, to process payments and health fund, workers compensation, CTP or NDIS claims, to communicate with your doctor, specialist or insurer about your care, and to run the practice, including quality improvement and meeting our legal obligations.
Who we share it with
We share health information only with people involved in your care or in paying for it: your GP, specialist or other health professionals, your health fund, insurer, NDIS plan manager or workers compensation insurer, and in an emergency. We do not sell or rent personal information. Appointment bookings are handled by Nookal, our practice management and online booking system, which stores information in Australia under its own privacy policy.
How we keep it safe
Your records are held in secure practice management software with access limited to the staff who need it. Paper records are kept securely in the clinic. We keep health records for the period required by law, which is at least seven years from your last consultation, or until a child patient turns 25.
Access and correction
You can ask to see the personal information we hold about you or ask us to correct it. Contact us at [email protected] or on (02) 4228 9299. We will respond within a reasonable time and may need to verify your identity first.
This website
This website does not use a contact form and does not collect personal information from you unless you contact us by phone or email. Online bookings are made on Nookal's site. We may use privacy-respecting analytics to see how the site is used; these do not identify you. Our website host may record standard server logs.
Complaints
If you are concerned about how we have handled your information, contact us first and we will try to resolve it. You can also contact the Office of the Australian Information Commissioner at oaic.gov.au or the NSW Information and Privacy Commission at ipc.nsw.gov.au.
